Job Description

Seeking a compassionate Nurse Practitioner or Physician Assistant to join our Palliative Care team.

The role involves independent and collaborative patient-centered care focused on symptom management and quality of life.

Responsibilities include assessments, care planning, regulatory visits, and transitions to hospice.

Requirements:
– Master’s in nursing (NP) or a bachelor’s degree and accredited PA program (PA)
– Active California NP, RN, or PA license
– Minimum two years of advanced practice experience

Skills include strong clinical assessment, interdisciplinary collaboration, excellent communication, and proficiency with electronic records. Preferred qualifications include palliative care experience and relevant board certifications.
