Job Description

The Post-Acute and Long-Term Care (PALTC) Advanced Practitioner (AP) is an in-person role for an APRN or PA with at least 2 years of experience, preferably in long-term care.

They work independently, managing comprehensive medical needs in PALTC settings to

  • improve care quality and outcomes,
  • enhance patient and family experience,
  • reduce unnecessary hospitalizations, and
  • lower costs.

  • Responsibilities include conducting examinations, diagnosing, treating, prescribing, and developing care plans, collaborating with facility staff and external providers, ensuring compliance, maintaining documentation, and promoting health and safety.

    Qualifications require a relevant degree, board certification, and an active APRN or PA license.

    The role involves strong communication, clinical decision-making, proficiency with EHR and telemedicine tools, and physical capability to respond quickly in emergencies.
