APRN/PA – Post-Acute and Long-Term Care (PALTC) – Advanced Practitioner (AP)

Job Description

The Post-Acute and Long-Term Care (PALTC) Advanced Practitioner (AP) is an in-person role for an APRN or PA with at least 2 years of experience, preferably in long-term care.

They work independently, managing comprehensive medical needs in PALTC settings to

improve care quality and outcomes,

enhance patient and family experience,

reduce unnecessary hospitalizations, and

lower costs.

Responsibilities include conducting examinations, diagnosing, treating, prescribing, and developing care plans, collaborating with facility staff and external providers, ensuring compliance, maintaining documentation, and promoting health and safety.

Qualifications require a relevant degree, board certification, and an active APRN or PA license.

The role involves strong communication, clinical decision-making, proficiency with EHR and telemedicine tools, and physical capability to respond quickly in emergencies.
