Seeking a Nurse Practitioner / Physician Assistant to provide outcome-based, innovative care for seniors in a community setting.
Responsibilities include assessing, managing, and coordinating healthcare for chronically ill geriatric participants, collaborating with physicians and interdisciplinary teams, and supporting clinical operations.
Ideal candidates are detail-oriented, analytical, and committed to high-quality, precise care within regulatory guidelines.
Schedule: Full-time, first shift, with hours set in consultation.
