Seeking a compassionate Nurse Practitioner or Physician Assistant for our Palliative Care team.

The role involves independent and collaborative patient-centered care focused on symptom management and quality of life, including assessments, care plans, regulatory visits, and hospice transitions.

Requirements include a Master’s in nursing or a relevant degree, active California licensure, and at least two years of advanced practice experience.

Skills needed are strong clinical assessment, interdisciplinary collaboration, excellent communication, and proficiency with electronic medical records. Valid driver’s license and reliable transportation are required.

Preferred qualifications include prior palliative care experience and relevant board certifications.

Position offers $59.52-$72.75/hour, 20-29 hours/week, with benefits such as health insurance, retirement plans, paid time off, and flexible scheduling.
