Job Description

The Post-Acute and Long-Term Care (PALTC) Advanced Practitioner (AP) is an APRN or PA working independently to manage patients in PALTC settings.
Responsibilities include conducting examinations, diagnosing, treating illnesses, prescribing medications, and developing care plans to improve care quality, patient outcomes, and reduce hospitalizations and costs.
They provide preventive services, collaborate with facility staff and external providers, ensure regulatory compliance, and participate in quality initiatives.
Strong clinical decision-making, communication, and proficiency with EHR and telemedicine are essential.
Qualifications include a relevant degree, at least 2 years of experience (preferably in long-term care), and active APRN or PA licensure.
Work involves in-person care in various facilities with full benefits offered.
