Job Description
Seeking a Nurse Practitioner for a 13-week locum tenens contract in Hoopa, CA, offering $90/hour for full-time outpatient adult primary care.
Provide comprehensive care including chronic disease management, preventive services, medication management, and referrals within a team-based clinic model. Work collaboratively with dedicated support staff, including care managers and medical assistants. Manage a patient panel with 30-minute appointments, seeing up to 15 patients daily during clinic hours (8:30 AM – 5:00 PM). Support services include on-site labs and X-ray with remote radiology reads. Emergency support involves stabilization and transfer if needed, with ambulance on-site. Ideal candidates have adult primary care experience, strong chronic disease skills, and comfort working in a rural setting.
This role offers a full-time schedule of approximately 40 hours per week, with potential for extension.
