Job Description
The Post-Acute and Long-Term Care (PALTC) Advanced Practitioner (AP) is an APRN or PA who independently manages patients in long-term care settings, collaborating with facility staff to enhance care quality, patient outcomes, and experience.
Responsibilities include conducting examinations, diagnosing, treating illnesses, prescribing medications, developing care plans, and providing preventive services. The AP works autonomously within scope, making clinical decisions and serving as the primary provider, while building strong relationships with facility leadership and interdisciplinary teams.
They ensure compliance with regulations, maintain accurate documentation, and participate in quality improvement. The role involves health and safety oversight, staff and patient education, and proficiency with EHR and telemedicine tools.
Qualifications include a relevant degree, 2+ years experience (preferably in long-term care), board certification, and an active APRN or PA license. The position is full-time, in-person, with comprehensive benefits and requires physical ability to respond quickly and move efficiently within the facility.
