Seeking a Nurse Practitioner for a 13-week locum tenens contract in Hoopa, CA, with a pay rate of $90/hour. This full-time role involves providing adult primary care in a team-based clinic, focusing on chronic disease management, preventive care, and referrals within clinic hours from 8:30 AM to 5:00 PM.

  • Work includes outpatient visits for conditions like diabetes, hypertension, CKD, and mental health, with up to 15 patients daily.
  • Support staff includes a dedicated Care Manager and Medical Assistant. The clinic has on-site labs, X-ray, and remote radiology reads.
