Job Description

The Post-Acute and Long-Term Care (PALTC) Advanced Practitioner (AP) is an in-person, full-time role for an APRN or Physician Assistant with at least 2 years of experience, ideally in long-term care.

  • Independently manage patient care in PALTC settings by conducting exams, diagnosing, treating, prescribing, and developing care plans to improve outcomes, reduce hospitalizations, and enhance patient and family experiences.
  • Collaborate with facility leadership and interdisciplinary teams, build strong relationships, and serve as a healthcare authority within the facility.
  • Ensure compliance with regulations, maintain accurate documentation, participate in quality initiatives, and promote infection control and health education.
  • Work independently with excellent clinical judgment, communication, and technology proficiency, including EHR and telemedicine tools.
