Join a physician-owned primary care group with multiple Rhode Island locations, dedicated to restoring patient-physician relationships and enhancing access to care.
Responsibilities:
- Manage adult patient panels, providing personalized chronic, acute, and preventive care.
- Conduct health screenings, vaccinations, and patient education on wellness strategies.
- Utilize data for care gap identification, support population health, and improve outcomes.
- Collaborate with interdisciplinary teams for integrated care.
- Maintain accurate EHR documentation, drive practice innovation, and meet clinical and financial goals.
Qualifications:
- MD or DO license, with experience in adult primary care or related fields.
- Strong communication skills and interest in value-based care.
