Join a physician-owned primary care group with multiple Rhode Island locations, dedicated to restoring patient-physician relationships and enhancing access to care.

Responsibilities:

• Manage adult patient panels, providing personalized chronic, acute, and preventive care.
• Conduct health screenings, vaccinations, and patient education on wellness strategies.
• Utilize data for care gap identification, support population health, and improve outcomes.
• Collaborate with interdisciplinary teams for integrated care.
• Maintain accurate EHR documentation, drive practice innovation, and meet clinical and financial goals.

Qualifications:

• MD or DO license, with experience in adult primary care or related fields.
• Strong communication skills and interest in value-based care.
