Seeking a Nurse Practitioner for a 13-week locum tenens contract in Hoopa, CA, with a pay rate of $90/hour. This full-time role involves adult primary care in a team-based clinic, focusing on chronic disease management, preventive care, and referrals.

The clinic has organized panels with dedicated support staff, including a pediatrician and acute care NP. Core duties include outpatient visits, managing conditions like diabetes and hypertension, and coordinating specialist referrals. The schedule is 8:30 AM – 5:00 PM, with up to 15 patients daily, and on-site lab and X-ray resources are available.

Ideal candidates have adult primary care experience, strong chronic disease skills, and comfort working in a rural setting. The team includes care managers and medical assistants, with emergency support available nearby. The role offers flexible work options, with 40 hours per week in person.
