Job Description
The Mental Health Therapist provides assessment, treatment, and collaborative support to clients across age groups, working within a medically integrated care team.
Deliver a minimum number of client visits weekly and conduct mental health assessments. Support clients in overcoming engagement barriers by coordinating with care team members for comprehensive wellness. Develop and implement personalized care plans, maintaining clear documentation. Participate in supervision, team meetings, and case conferences, offering and seeking consultation as needed. Provide evidence-based clinical treatment and engage with relevant systems and participants to ensure cohesive care. Complete required training, supervision, and consultation activities.
