Job Description
The Post-Acute and Long-Term Care (PALTC) Advanced Practitioner is an APRN or Physician Assistant working independently to manage patients in PALTC settings.
Provide comprehensive assessments, diagnose, treat illnesses, prescribe medications, and develop care plans. Collaborate with facility leadership and interdisciplinary teams to ensure holistic care and effective communication. Maintain compliance with regulations, document accurately, and participate in quality improvement efforts. Ensure patient safety through infection control, health education, and adherence to best practices. Work independently, make clinical decisions, and respond swiftly in emergencies. Utilize EHR and telemedicine tools effectively.
Qualifications include a relevant degree, 2+ years of experience (preferably in long-term care), and board certification.
