Job Description

We are seeking an experienced Nurse Practitioner to provide primary care in Skilled Nursing Facilities, LTC, and ALFs.

  • Deliver comprehensive assessments, manage acute and chronic conditions, and develop treatment plans.
  • Order diagnostics, prescribe medications, and educate patients, families, and staff.
  • Collaborate with care teams to ensure seamless patient care and support quality initiatives.

    • Qualifications include a Minnesota NP license, board certification, and preferably 1+ year in post-acute settings. Strong geriatric and primary care skills are essential.
