Home-Based Therapist

Job Description

We are seeking a compassionate *Home-Based Therapist* to provide intensive in-home services to children, adolescents, and their families in Bad Axe, Michigan.

Provide clinical assessments and develop individualized family service plans.

Conduct regular home visits to teach parenting, model behavior management, and offer therapy.

Maintain consistent contact with families and coordinate with community agencies.

Deliver crisis stabilization and participate in on-call rotations.

Document progress and advocate for family resources.

Engage in team meetings, community education, and program planning.

Qualifications: Master’s degree in Social Work, Counseling, or Psychology with Michigan licensure (e.g., LLMSW, LMSW). Minimum 2 years of experience preferred, valid driver’s license required, and willingness to complete Infant Mental Health endorsement. Flexibility for evening/weekend hours is necessary.
