Community Health is a primary care network offering comprehensive wellness, dental, behavioral health, and pediatric services across eight locations in Vermont.

About the Role: A Licensed Mental Health Clinician specializing in pediatrics will provide mental health treatment, support, and advocacy for children and their families in primary care settings.

Key Functions:

  • Offer emotional support and counseling to pediatric patients and families.
  • Assess social, environmental, and psychosocial needs, including social determinants of health.
  • Develop individualized care plans with multidisciplinary teams.
  • Conduct therapy sessions using evidence-based techniques.
  • Provide education on coping, disease management, and community resources.
  • Maintain accurate documentation and stay current with best practices.
  • Collaborate closely with healthcare professionals to address holistic patient needs.

  • Qualifications: Master’s degree in social work, counseling, or related field, licensure required, pediatric or primary care experience preferred. Strong communication, empathy, and teamwork skills are essential.
