Job Description

We are seeking a qualified Nurse Practitioner (Skilled Nursing) to join our team and serve as a clinical leader in a Skilled Nursing Facility setting.

  • Responsibilities include managing admissions, discharges, monthly checks, and assessments for changes in residents’ conditions.
  • Performing advanced care planning, comprehensive evaluations, and prescribing treatments.
  • Collaborate with the care team to ensure quality resident care.
