Seeking Board Certified Geriatricians, Internists, and Family Practitioners to provide comprehensive on-site medical care in post-acute, rehabilitation, long-term care, and assisted living facilities.
Deliver face-to-face primary care services and participate in educational sessions and clinical reviews. Collaborate with Nurse Practitioners and Physician Assistants. Serve a diverse patient population, from short-term rehab to complex geriatric cases.
