Job Description

We are seeking an experienced Nurse Practitioner to provide comprehensive primary care in skilled nursing, long-term care, and assisted living facilities.

  • Deliver patient assessments, diagnose, and manage acute and chronic conditions.
  • Order labs, develop treatment plans, prescribe medications, and educate patients and staff.
  • Collaborate with care teams to ensure seamless, quality care.
