Nurse Practitioner – Geriatric Primary Care (SNF, LTC, ALF)

Job Description

We are seeking an experienced Nurse Practitioner to provide comprehensive primary care in Skilled Nursing Facilities, LTC centers, and Assisted Living Facilities.

Perform patient assessments, order diagnostics, develop treatment plans, and manage acute and chronic conditions.

Prescribe medications, educate patients and staff, and collaborate with care teams for seamless service.

Offer autonomy with flexible scheduling, minimal on-call duties, and a supportive environment.

Qualifications: Valid Minnesota NP license, board certification, and preferred experience in geriatric or post-acute care.
