Job Description

Advanced Practice Provider position providing outpatient family medicine care in Prineville, OR.

  • Under supervision, delivers comprehensive medical evaluations, diagnoses, treatments, and documentation for patients of all ages within a Medical Home model.

  • Coordinates with multidisciplinary team members, including behavioral health, pharmacy, and care management, to develop and monitor personalized treatment plans.

  • Participates in clinic call rotation, providing after-hours coverage, with backup support from physicians.

  • Practices evidence-based care, prescribes medications, interprets diagnostics, and educates patients.

  • Maintains professionalism, compliance, and current licensure, and collaborates with team members to improve quality outcomes.

  • Requires Master’s degree in Nursing or Physician Assistant Studies, valid licensure, certification, and outpatient care experience, preferably in Family Medicine.

  • Full-time role with benefits, including health, dental, vision, retirement, and paid time off, with relocation to Prineville, OR.
