Seeking an Advanced Practice Provider to deliver comprehensive outpatient family medicine care in Prineville, OR.
Under supervision, provide evaluation, diagnosis, treatment, and documentation for patients of all ages. Coordinate with multidisciplinary teams within a Medical Home model to optimize patient outcomes and costs. Develop and monitor treatment plans integrating behavioral health, pharmacy, therapy, nutrition, or surgery, and refer patients to specialists as needed. Participate in clinic call rotation, including evenings, weekends, and holidays. Practice evidence-based family medicine, ensuring quality, safety, and patient-centered care. Maintain accurate documentation, interpret diagnostic tests, and educate patients on health management. Support quality improvement initiatives and uphold professional standards and compliance. Qualifications include a Master’s in Physician Assistant Studies or Nursing, valid licensure, certification, and experience in outpatient family medicine. Preferred candidates have 2+ years of experience, familiarity with Medical Home models, and Epic EHR skills.
