Job Description
The Nurse Practitioner (CRNP) provides primary care and case management for participants in a community-based program serving elderly individuals in western Pennsylvania.
Manage health, disease prevention, and wellness in collaboration with interdisciplinary teams. Develop personalized Plans of Care aligned with participant goals. Require licensed RN with advanced certification, experience with elderly populations, and knowledge of community health and care management models. Work hours are typically 8:30AM to 5:00PM, with a focus on holistic, person-centered care.
