Job Description

The Nurse Practitioner (CRNP) provides primary care and case management for participants in a community-based program serving elderly individuals in western Pennsylvania.

  • Manage health, disease prevention, and wellness in collaboration with interdisciplinary teams.
  • Develop personalized Plans of Care aligned with participant goals.
  • Require licensed RN with advanced certification, experience with elderly populations, and knowledge of community health and care management models.
  • Work hours are typically 8:30AM to 5:00PM, with a focus on holistic, person-centered care.
