Job Description
The Advanced Practice Provider (APP) delivers comprehensive outpatient care across all ages under supervision, including evaluation, diagnosis, treatment, and documentation. They coordinate services within a Medical Home model, creating treatment plans incorporating behavioral health, pharmacy, therapy, nutrition, and surgery, and monitor ongoing patient progress.
Work involves at least 36 hours of patient contact weekly, with scheduled days varying, and participation in shared call rotation covering evenings, weekends, and holidays, supported by backup physicians.
In a team-based environment, the APP practices evidence-based family medicine, collaborates with multidisciplinary teams, prescribes medications, interprets diagnostics, educates patients, and supports quality improvement initiatives. They uphold professionalism, confidentiality, and compliance with regulations, maintaining current licenses and certifications.
Qualifications include a Master’s degree in Physician Assistant Studies or Nursing, valid licensure, certification, and experience in outpatient family medicine, with preferred advanced degrees and years of practice.
