Job Description
The Advanced Practice Provider (APP) provides comprehensive outpatient care in a Family Medicine Medical Home setting, under supervision of a physician and Clinical Division Director.
Responsibilities include evaluation, diagnosis, treatment, medication prescribing, coordinating with Behavioral Health, pharmacy, and other services, and monitoring patient progress.
Work involves minimum 40 hours/week with scheduled days varying, and participation in a shared call rotation including evenings, weekends, and holidays.
Supported by a multidisciplinary team, the APP delivers evidence-based, personalized care, promotes quality health outcomes, and adheres to professional standards, policies, and regulations.
Qualifications include a Master’s in Physician Assistant Studies or Nursing, valid licenses, certifications (e.g., NCCPA, AANC), and experience in outpatient Family Medicine, EHR use, and team collaboration. Preferred candidates have 2+ years’ experience, familiarity with the Medical Home model, and Epic EHR.
