Licensed Social Worker – Hospice

Job Description

We are seeking a compassionate and professional Licensed Social Worker to join our hospice team in the Charleston, SC area.

In this role, you will

interact with diverse individuals and implement hospice policies and procedures

perform assessments and social evaluations, including family dynamics and caregiver abilities

identify high-risk cases for suicide, neglect, or abuse and develop intervention plans

collaborate as part of an interdisciplinary team to provide patient care

.

Qualifications include effective communication, integrity, understanding of hospice philosophy, ability to travel within the community, proficiency with Microsoft Office, and licensure in South Carolina. A valid driver’s license and auto insurance are required.
