Job Description

Seeking a Speech Language Pathologist for our Home Care team to provide personalized evaluations and treatments in patients’ homes, focusing on communication, cognition, and swallowing for safety and independence.

Responsibilities include interpreting medical prescriptions, conducting assessments, developing evidence-based treatment plans, adjusting strategies based on progress, and documenting care per regulations.

The role involves educating patients and caregivers, collaborating with interdisciplinary teams, and staying current with clinical best practices.

Qualifications include a Master’s in Speech-Language Pathology, CCC or CFY, NY State licensure, and CPR certification; experience with dysphagia and neurogenic disorders is preferred.
