Job Description

The Licensed Clinical Social Worker (LCSW) provides evidence-based psychotherapy, including individual, group, and family counseling, within an integrated primary care team.

They conduct assessments, diagnosis, treatment planning, and crisis intervention, collaborating with medical providers and external partners to support patients’ behavioral and physical health.

The role involves documenting sessions in the EHR, developing self-management plans, and participating in team meetings, trainings, and quality improvement initiatives.

Work is primarily in an office setting with some regional travel, requiring a Masters-level social work credential and at least 2 years of relevant experience.
