Licensed Clinical Social Worker (LCSW) – PART-TIME

Job Description

The Licensed Clinical Social Worker (LCSW) provides evidence-based psychotherapy, including individual, group, and family counseling, to patients in a primary care setting.

They conduct assessments, develop diagnosis and treatment plans, and support patients in managing behavioral health and physical health conditions.

Working collaboratively with the primary care team and external partners, they offer crisis intervention, referrals, and health education.

Responsibilities include documenting sessions, participating in team meetings, contributing to quality improvement, and providing training on behavioral health topics.

This role requires a Master’s degree in social work or related field, with at least 2 years of experience preferred, and involves part-time, in-person work with benefits offered.
