Job Description

Seeking a Speech Language Pathologist for our Home Care team to provide personalized evaluation and treatment in patients’ homes, focusing on communication, cognition, and swallowing to promote safety and independence.

Responsibilities include interpreting prescriptions, conducting assessments, developing evidence-based treatment plans, adjusting strategies based on progress, documenting care, and educating patients and caregivers. Collaboration with interdisciplinary teams and participation in care planning are essential.

Qualifications include a Master’s degree, CCC or CFY, NY State license, and CPR certification; experience with dysphagia and neurogenic disorders is preferred.
