Job Description
We are seeking an experienced Diabetic Practice Nurse to join a friendly, city-centre GP surgery on a part-time basis (23-30 hours/week).
**Key duties include:**
Leading and delivering structured diabetes reviews via face-to-face and phone consultations Optimising glycaemic control and cardiovascular risk management Initiating and titrating diabetes medications, including injectables Providing patient education and promoting self-management Maintaining QOF performance and liaising with multidisciplinary teams Supporting audits and quality improvement initiatives**Candidate requirements:**
Registered Nurse with NMC registration and UK nursing experience (minimum 2 years) Diabetes qualification or working towards one, with confidence in insulin management Organised, proactive, and team-oriented with excellent communication skills Able to travel between surgeries in Chichester and Bosham**We offer:**
