Job Description

We are seeking an experienced Diabetic Practice Nurse to join a friendly, city-centre GP surgery in Chichester, working 23-30 hours per week.

Responsibilities include leading diabetes reviews, optimizing glycaemic control, initiating and titrating medications, delivering patient education, and supporting quality improvement initiatives.

Ideal candidates are registered nurses with NMC registration, primary care diabetes experience, and a relevant qualification. Confidence in insulin management and strong team collaboration skills are essential.
