Job Description

We are seeking an experienced Diabetic Practice Nurse to join our friendly, city-centre GP surgery on a part-time basis (23-30 hours/week).

  • Lead and conduct structured diabetes reviews, both face-to-face and remotely
  • Optimize glycaemic control and manage cardiovascular risks
  • Initiate and adjust diabetes medications, including injectables
  • Provide patient education and support self-management
  • Maintain QOF performance and collaborate with MDT and community teams
  • Support audits and quality improvement projects

    • Requirements include UK NMC registration, primary care diabetes experience, and a relevant qualification (or working towards one). The role involves travel between two sites and offers benefits such as NHS pension, uniform, parking permit, and professional development opportunities.
