Job Description

Seeking an experienced Advanced Practice Provider (PA or NP) passionate about compassionate, high-touch wound care in patients’ homes.

This full-time, salaried role offers flexible Monday-Friday hours, no on-call, weekends, evenings, or holidays, with daily local travel reimbursed and all necessary equipment provided.

Responsibilities include assessing patients, developing individualized care plans, performing advanced treatments, managing medications, and educating patients and staff. The ideal candidate has at least one year of wound care experience, current licensure in Minnesota, and the ability to travel approximately 75% of the time.
