Job Description
The Advanced Practice Provider (APP) provides mobile wound care across various settings including independent living, assisted living, and long-term care facilities. They manage patient care from history intake to assessment, diagnosis, treatment, and follow-up, collaborating with a multidisciplinary team to ensure high-quality outcomes.
Requires licensure as an RN and APN or PA, with certification as a Nurse Practitioner, Clinical Nurse Specialist, or Physician Assistant.
Must have advanced degree (Master’s or Doctorate), prescriptive authority, and current BLS (and ACLS preferred).
Minimum one year of clinical and wound care experience; proficiency in wound debridement and skin substitutes is preferred.
Strong communication, organizational, and independent working skills are essential.
Responsible for managing patient caseload, providing education, ensuring compliance, and participating in team and organizational development activities.
Travel is required for home visits and LTCs, with a full-time position offering comprehensive benefits and a salary range of $130,500 to $200,000 annually.
