Job Description

Seeking an Advanced Practice Provider (PA or NP) passionate about compassionate, high-touch wound care in a mobile, home-based setting.

Responsibilities include conducting comprehensive assessments, developing personalized care plans, performing advanced treatments, managing medications, and collaborating with healthcare teams to ensure optimal patient outcomes.

Qualifications:

Graduate of an accredited nurse practitioner or physician assistant program

At least 1 year of experience in wound care or relevant specialties

Board certification through AANP, ANCC, or NCCPA

Current, unrestricted license in Minnesota and ability to obtain DEA and prescriptive authority

Benefits include a flexible schedule (Monday-Friday, no evenings/weekends), full benefits, mileage reimbursement, and a salary range of $130,500–$200,000.

Ideal candidates are experienced, committed to patient-centered care, and willing to travel approximately 75% locally to patient homes.
