This full-time role offers a flexible schedule with 2 days remote work and travel across Iowa, including mileage reimbursement.
We seek a compassionate Advanced Provider (Nurse Practitioner or Physician Assistant) to conduct wellness exams and manage care in assisted living and nursing home settings.
Key duties include performing assessments, managing chronic and acute conditions, documenting in EMR/EHR systems, providing patient education, and collaborating with healthcare teams.
