Job Description

We are seeking a compassionate Autonomous Nurse Practitioner to join our mobile healthcare team, providing high-quality patient care across various settings.

Conduct comprehensive patient assessments, including physical exams and health histories.

Develop and implement individualized care plans.

Manage patients with chronic conditions and educate patients and families on health management and wellness.

Ensure HIPAA compliance and maintain confidentiality.

Participate in utilization management and adhere to infection control protocols.

Collaborate with healthcare professionals to coordinate care.

Candidates should have strong experience in wound and critical care, proficiency in medication administration, and familiarity with behavioral health issues.
