Job Description

The Post-Acute and Long-Term Care (PALTC) Advanced Practitioner (AP) is an independently practicing APRN or PA who collaborates with facility staff to manage patients’ comprehensive medical needs.

Responsibilities include conducting examinations, diagnosing and treating illnesses, developing care plans, and providing preventive services. The AP makes autonomous clinical decisions within scope, builds collaborative relationships with facility leadership and interdisciplinary teams, and ensures compliance with regulations.

They oversee health and safety protocols, educate staff and patients, and maintain detailed documentation. The role requires strong communication, leadership, and technology skills, as well as the ability to work independently and respond swiftly in emergencies.

Qualifications include a relevant degree, at least 2 years of experience (long-term care preferred), and board certification. The position is full-time, in-person, with benefits such as health insurance, retirement plans, and paid time off.
