Job Description
The Post-Acute and Long-Term Care (PALTC) Advanced Practitioner (AP) is an independent Advanced Practice Registered Nurse (APRN) or Physician Assistant (PA) working in PALTC facilities.

They work independently within scope, ensuring compliance, maintaining accurate records, and participating in quality initiatives.
The role requires strong communication, clinical decision-making, and proficiency with health technology. Candidates should have a relevant degree, at least 2 years of experience, board certification, and the ability to work independently in in-person, statewide PALTC settings.
