Job Description
The Post-Acute and Long-Term Care (PALTC) Advanced Practitioner (AP) is an APRN or PA working independently to manage patients in PALTC settings.
They aim to improve care quality, patient outcomes, and experience while reducing hospitalizations and costs.
- Patient Care: Conduct exams, diagnose, treat illnesses, prescribe medications, and provide preventive services in collaboration with staff.
- Self-directed Practice: Independently assess and treat within scope, making clinical decisions and escalating as needed.
- Collaborative Relationships: Build partnerships with facility leadership and interdisciplinary teams to coordinate care and communicate with families.
- Compliance & Documentation: Ensure regulatory adherence, maintain records, and participate in quality initiatives.
- Health & Safety: Follow infection control protocols and educate staff and patients on health promotion.
- Work Capabilities & Skills: Work independently, demonstrate strong clinical decision-making, communication, and leadership skills.
- Technology & Physical Requirements: Proficient with EHR and telemedicine; able to respond quickly and move safely.
Qualifications include a relevant degree (e.g., MSN, DNP), at least 2 years of experience (long-term care preferred), and board certification. The role is full-time, in-person, across statewide facilities, offering comprehensive benefits.
