Job Description

The Post-Acute and Long-Term Care (PALTC) Advanced Practitioner (AP) is an APRN or PA working independently in PALTC facilities to enhance care quality, patient outcomes, and experience while reducing hospitalizations and costs.

  • Patient Care: Conduct exams, diagnose/treat illnesses, develop care plans, prescribe medications, and provide preventive services.
  • Self-directed Practice: Independently assess and treat within scope, making clinical decisions and escalating as needed.
  • Collaborative Relationships: Work with facility leadership, interdisciplinary teams, patients, and families to coordinate care.
  • Compliance & Documentation: Maintain accurate records, ensure regulatory adherence, and participate in quality initiatives.
  • Health & Safety Oversight: Follow infection protocols, promote health education, and ensure patient safety.
  • Work Skills: Work independently, demonstrate strong clinical judgment, communication, and leadership skills.
  • Technology: Proficient in EHR systems, telemedicine, and virtual care platforms.

Qualifications include a relevant degree, at least 2 years of experience (preferably in long-term care), and active APRN or PA licensure. This full-time role involves in-person work across statewide facilities, offering various benefits.
