Job Description

The Post-Acute and Long-Term Care (PALTC) Advanced Practitioner (AP) is an APRN or Physician Assistant providing independent, comprehensive patient care in long-term care facilities.

Responsibilities include conducting assessments, diagnosing, treating illnesses, prescribing medications, and developing care plans to improve outcomes, reduce hospitalizations, and enhance patient and family experience.

They work autonomously, collaborate with facility leadership and interdisciplinary teams, ensure compliance with regulations, maintain accurate documentation, and promote health and safety protocols.

Qualifications include a relevant degree (BS-PAS/MPAS/DMSc/MSN-NP/DNP), at least 2 years of experience (long-term care preferred), active APRN or PA license, and board certification. The role requires strong clinical, communication, and technology skills, with a full-time, in-person work environment offering comprehensive benefits.
