Job Description
The Post-Acute and Long-Term Care (PALTC) Advanced Practitioner (AP) is an in-person, full-time role for an APRN or Physician Assistant with at least 2 years of experience, ideally in long-term care.
Independently manage patient care in PALTC settings by conducting exams, diagnosing, treating, prescribing, and developing care plans to improve outcomes, reduce hospitalizations, and enhance patient and family experiences. Collaborate with facility leadership and interdisciplinary teams, build strong relationships, and serve as a healthcare authority within the facility. Ensure compliance with regulations, maintain accurate documentation, participate in quality initiatives, and promote infection control and health education. Work independently with excellent clinical judgment, communication, and technology proficiency, including EHR and telemedicine tools. Qualifications include a relevant degree, licensure, board certification, and experience in long-term care preferred.
