Job Description

The Post-Acute and Long-Term Care (PALTC) Advanced Practitioner (AP) is an experienced APRN or PA working independently to deliver comprehensive care in PALTC settings.
Responsibilities include conducting physical exams, diagnosing and treating illnesses, developing care plans, and providing preventive services.
The AP collaborates with facility leadership and interdisciplinary teams, building strong relationships to ensure holistic patient care, while maintaining compliance with regulations and accurate documentation.
They independently manage patient care, make clinical decisions, and serve as a primary provider within the facility.
Additional duties involve infection control, health education, and utilizing electronic health records and telemedicine tools.
Qualifications include a relevant degree, at least 2 years of experience (long-term care preferred), and board certification.
This full-time role requires excellent communication, decision-making skills, and the ability to work in person across various facilities, with benefits including health insurance, retirement plans, and paid time off.
