Job Description
The Post-Acute and Long-Term Care (PALTC) Advanced Practitioner (AP) is an APRN or Physician Assistant working independently in long-term care facilities to enhance patient outcomes, improve care experiences, reduce hospitalizations, and lower costs.
Responsibilities include conducting exams, diagnosing, treating illnesses, managing chronic conditions, providing preventive care, and developing care plans.
The AP works autonomously within scope, making clinical decisions and collaborating with facility leadership and interdisciplinary teams to ensure holistic care.
Key duties involve ensuring regulatory compliance, maintaining accurate documentation, participating in quality improvement, and promoting health and safety protocols.
Strong communication, leadership, and clinical decision-making skills are essential, along with proficiency in EHR and telemedicine tools. The role requires physical stamina and the ability to work independently in a fast-paced setting.
Qualifications include a relevant degree, at least 2 years of experience (long-term care preferred), and active APRN or PA licensure. The position is full-time, in-person, across statewide facilities, offering comprehensive benefits.
