The Post-Acute and Long-Term Care (PALTC) Advanced Practitioner (AP) is an APRN or PA working independently to provide comprehensive medical care in long-term care facilities.
Responsibilities include conducting physical exams, diagnosing and treating illnesses, managing chronic conditions, and providing preventive care. Work autonomously within scope, making clinical decisions and acting as the primary provider, while collaborating with facility leadership and interdisciplinary teams. Ensure compliance with regulations, maintain accurate documentation, and participate in quality improvement and safety initiatives. Educate staff and patients on health promotion, and utilize EHR and telemedicine tools effectively. Qualifications include a relevant degree, 2+ years of experience (preferably in long-term care), and active APRN or PA licensure.
