Job Description

The Post-Acute and Long-Term Care (PALTC) Advanced Practitioner (AP) is an APRN or PA working independently to manage patient care in long-term care settings.

Their responsibilities include conducting exams, diagnosing, treating, prescribing medications, and developing care plans to improve outcomes, enhance patient experiences, reduce hospitalizations, and lower costs.

They collaborate with facility staff, build relationships with interdisciplinary teams, ensure regulatory compliance, maintain accurate documentation, and participate in quality initiatives.

The role requires strong clinical decision-making, independent work capability, excellent communication, proficiency with EHR and telemedicine, and physical readiness to respond quickly.

Qualifications include a relevant degree, 2+ years of experience (preferably in long-term care), and board certification.
