Job Description
The Post-Acute and Long-Term Care (PALTC) Advanced Practitioner (AP) is an APRN or Physician Assistant responsible for delivering independent, comprehensive medical care in PALTC settings to enhance quality outcomes, patient experience, and reduce hospitalizations and costs.
Key duties include conducting exams, diagnosing, treating, prescribing, and managing chronic and acute conditions, providing preventive care, and collaborating with facility staff and external providers. The AP works autonomously within scope, ensuring compliance with regulations, maintaining accurate documentation, and participating in quality improvement.
Additional responsibilities involve infection control, staff and patient education, and utilizing electronic health records and telemedicine tools. Strong communication, clinical decision-making, and leadership skills are essential, along with the ability to work independently and respond swiftly in emergencies. Qualifications include a relevant degree, at least 2 years of experience (preferably in long-term care), and appropriate certification and licensure.
