Job Description
The Post-Acute and Long-Term Care (PALTC) Advanced Practitioner (AP) is an in-person, full-time role for an APRN or PA with at least 2 years of experience, ideally in long-term care.
Provide independent, comprehensive medical care, including assessments, diagnoses, treatments, and preventive services in PALTC settings. Collaborate with facility leadership and interdisciplinary teams to ensure holistic patient care, while building strong relationships with patients and families. Ensure compliance with regulations, maintain accurate documentation, and participate in quality improvement efforts. Manage health and safety protocols, promote health education, and work proficiently with EHR and telemedicine tools. Demonstrate strong clinical decision-making, communication, leadership, and the ability to work autonomously in a fast-paced environment.
