Job Description

The Post-Acute and Long-Term Care (PALTC) Advanced Practitioner (AP) is an in-person, full-time role for an APRN or PA with at least 2 years of experience, ideally in long-term care.

  • Provide independent, comprehensive medical care, including assessments, diagnoses, treatments, and preventive services in PALTC settings.
  • Collaborate with facility leadership and interdisciplinary teams to ensure holistic patient care, while building strong relationships with patients and families.
  • Ensure compliance with regulations, maintain accurate documentation, and participate in quality improvement efforts.
  • Manage health and safety protocols, promote health education, and work proficiently with EHR and telemedicine tools.
  • Demonstrate strong clinical decision-making, communication, leadership, and the ability to work autonomously in a fast-paced environment.
